Physicals
Immigration, DOT, and school bus driver physicals are available. Immigration physicals are available at our Onley center. Call the Onley center at (757) 787-7374 to make an appointment. DOT and school bus driver physicals are available at each center and appointments can be made by calling the center directly – see the Locations tab for center information. Be prepared! For immigration physicals please bring the most recent version of Form I-693 you can find here: https://www.uscis.gov/i-693 to your exam. Complete Part 1 of Form I-693 but do not sign until instructed to do so. For school bus driver physicals you can find the form here. Physicals for school are available as well. Children playing school sports will need this form completed before trying out for a school team or participating in conditioning activities sponsored by the school. Students entering pre-kindergarten or kindergarten will need this form.
