ESRHS Named Health Center Quality Leader by HRSA
We were awarded a Quality Improvement Award by the Health Resources & Services Administration (HRSA) again in 2020. HRSA provides these awards to promote optimization of overall quality, efficiency, and value of the health care services provided by the nation’s health centers. We were recognized as a “gold” Health Center Quality Leader – Health Center Quality Leaders achieved the best overall clinical performance among all health centers and were recognized in the following tiers: Gold (top 10%), Silver (top 11-20%), or Bronze top (21-30%) for CQMs in 2019.
Patient Centered Medical Home
The National Committee for Quality Assurance (NCQA) has bestowed Patient-Centered Medical Home Recognition to Eastern Shore Rural Health System, Inc. Health care organizations are recognized for using proven methods delivered in full coordination with all the patient’s health care providers. This is provided in a setting in which the patient and provider work as a team. Each of Rural Health’s four medical sites – Atlantic, Chincoteague Island, Eastville and Onley – has been recognized as patient centered medical homes.

- Support your health needs and goals.
Patient-centered medical homes lead to better care, lower costs, and improved satisfaction with your health care. Each patient has an ongoing relationship with a personal physician who leads a care team at a single location. That team takes responsibility for patient’s needs, their care, and arranges for appropriate care with other qualified clinicians.
